Cubicle Assignments

Cubicles will be assigned using the candidate sequential number (located on your CDCA Candidate Identification/Information Card) by the College of Dentistry.  The assignments will be posted the evening prior to the examination on the window outside of Clinic Administration.

Health and Safety Expectations

All candidates are expected to be in compliance with the College of Dentistry immunization requirements that include:

  1. Rubella: (German Measles): Immunization or presence of serum antibody.
  2. Rubeola: (Measles): Immunization or presence of serum antibody.
  3. Hepatitis B: A completed Hepatitis B vaccination series of three injections. Individuals completing the series since December 1997 must have evidence of serum antibody.
  4. Varicella Zoster: (Chicken Pox): Immunization or presence of serum antibody.
  5. Diphtheria: Immunization within the last 10 years.
  6. Tetanus:  Immunization within the last 10 years.
  7. Tuberculosis: A negative skin test within one year of the examination.  Anyone with a history of a previously positive skin test must have an evaluation for disease by a qualified nurse or physician and provide a letter indicating a non-infectious state.

Fees

The Clinic Usage Fees, which apply to non-college of dentistry candidates, is $100 per day.

Instrument rental and supplies fee, which apply to non-college of dentistry candidates, is $100.00 per day

All fees may be paid by cashier's check made payable to The Ohio State University only during business hours in the Clinic Admin Office Rm. 1130. Payment is required prior to start of exam.

Infection Control

Each cubicle is supplied with CaviWipes to wipe down large surface areas of the cubicle such as counter top, dental chair and operator stool.  Barriers are available in the Supply Side Rooms. Birex is available for impression material.

Provided either in person or via zoom. Contact Dr. Reyes-Perez if you do not receive an email with further information.
